Healthcare providers can follow and monitor all their patients in a dashboard, where patients that need urgent attention and at risk patients are highlighted including individual patient statistics.

To be able to use the Altus Care app, your healthcare provider needs to assign you a treatment plan. Without this, the app can't be activated.
