Alzheimer Society Care Partner Description
The Alzheimer Society of Toronto Care Partner App, previously known as the Dementia Talk App - an award winning mobile application created by the Reitman Centre at Mount Sinai Hospital. It was designed to empower caregivers of people with dementia to manage care at home. The Alzheimer Society of Toronto’s mission is to alleviate the personal and social consequences of Alzheimer’s disease and other dementias and to promote research.
Caring for someone living with dementia can be a long journey with many twists and turns. As the brain changes, the person’s interpretation of the world changes and can often lead to responsive behaviours that can be challenging for their caregiver to know how to respond. The Care Partner mobile application uses tools and resources to support you as the caregiver to someone living with dementia for the promotion of everyone's wellbeing.
This app will allow the caregiver to:
- Track behaviours of concern to support identification of patterns or possible triggers
- Develop a care plan to address behaviours of concern
- Coordinate care with others such as family, friends and other supports
- Access resources to support caregiver wellbeing
