2. Upload a readable copy of your Driver's License, State ID card, or passport. Do not obscure any information except your identification numbers from the ID card.
3. Upload a clear and recent photo of yourself.
4. Include your full name and email on your application. Your information is secure and confidential. Your name will be on your certificate and we will send your certificate via email once you have been verified.
